Terms of Service

Applies to the Signature Tox Membership ($50/month) at Beholding Beauty Aesthetics

1. Monthly Membership Fee
* The Signature Tox Membership is $50 per month, billed automatically.
* Membership fees do not convert to service credit, prepaid balances, or account wallets.
* Membership fees are non-refundable and have no cash value.

2. Membership Term & Renewal
* A 3-month minimum commitment is required at enrollment.
* Memberships may not be canceled or paused during the initial 3-month commitment period.
* After the initial commitment, the membership continues on a month-to-month basis unless canceled.
* Cancellation requires 30 days’ notice prior to the next billing cycle.
* Membership benefits end at the conclusion of the final billing period.

3. Member Pricing, Benefits & Usage
* Membership benefits apply only to the enrolled member and are non-transferable.
* Preferred member pricing includes $225 per treatment area for Dysport, Jeuveau, or Xeomin.
* Botox pricing is not included in membership pricing and is billed separately at standard rates.
* Membership pricing, discounts, and perks cannot be combined with packages, flash sales, or other promotions.
* Complimentary perks (including sign-on gifts, birthday benefits, and anniversary services) are available only after the initial 3-month commitment and while the membership remains active and in good standing.

4. Payment Policies
* Accepted payment methods include Visa, Mastercard, American Express, and Discover.
* Membership dues must be paid via a valid card on file and cannot be paid using store credit or discounts.
* If a payment method is declined or becomes past due for more than 30 days, the membership may be suspended or canceled.

5. No Beauty Bank or Stored Credit
* The Signature Tox Membership does not include a Beauty Bank, wallet, or stored account credit.
* Monthly membership fees provide access to member-only pricing and benefits, not prepaid services.
* Unused benefits do not roll over and have no cash value.

6. Appointment Availability
* Priority scheduling is offered to active Signature Tox Members.
* Specific appointment times cannot be guaranteed; however, enhanced efforts will be made to accommodate preferred scheduling based on availability.

7. Member Responsibility & Medical Eligibility
* Members are responsible for maintaining accurate billing and contact information, including name, phone number, date of birth, and payment details.
* Members agree to notify Beholding Beauty Aesthetics promptly of any changes.
* All treatments are subject to medical evaluation, eligibility, and provider approval.
* Beholding Beauty Aesthetics reserves the right to decline or discontinue services if medically inappropriate or if membership terms are violated.

8. Practice Rights
* Beholding Beauty Aesthetics reserves the right to modify membership pricing, benefits, or terms with prior notice.
* Membership enrollment may be limited to preserve appointment availability and quality of care.
